HSA/FSA Card Users
Please note: MassageBook uses Merchant Classification Code (MCC) 8099 ‘Medical Services and Health Practitioners, Not Elsewhere Classified.’ HSA/FSA cards may have restrictions as to what Merchant Classification Codes are accepted. If your card is not covered under MCC 8099, I recommend that you contact your card issuer in order to determine the options moving forward, as coverage is dependent upon the specific HSA/FSA plan.
First Time Clients
Appointments with first-time clients include a 10-15 minute information session to go over the Client Intake Form and Medical History you filled out online. We will cover the steps involved in a session and set your goals for the session. It will then be followed by the scheduled massage. Please undress to your level of comfort for any Swedish or prenatal massage. Reiki, Thai & TMJD are all performed fully clothed. Draping will be used during the session – only the area being worked on will be uncovered. In general, massage is given while you are unclothed. However, you may choose to wear undergarments. You will be covered with a top sheet throughout your session. After the session, we will have time to make sure we accomplished what was set forth at the beginning of the appointment.
The need to have medical information of the client:
It is necessary to have information about your medical history so I can plan for a massage session that is safe and effective for you. Knowing this information is also helpful, as certain conditions like systemic contagious or infectious diseases (including the common cold), acute conditions requiring first aid or medical attention, severe unstable, hypertension, significant fever are contraindicated for massage.
Please keep me updated on any changes in your medical profile and understand that there shall be no liability on my part should you forget to do so.
If you need to cancel or reschedule your appointment, 24-hour advance notice is required, so that I may fill your session time with another client. “No Shows” and same day cancellations will be charged the full session fee. I understand that things happen in everyone’s life, however, please call or text as soon as possible to cancel and we can avoid cancellation fees if you can reschedule within 10 days of your original appointment.
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, I will determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session. Out of respect and consideration for me and other customers, please plan accordingly and be on time.
